Below is the 2010 SBA Team and Player Agreement, Contract and Waiver. You can print this page off, or there will be contracts at the location your division will be playing at. This must be turned in with all entry fees to the location for the division your respective team will be playing at. It is DUE March 15, 2010 with all fees. You can pay online, at a rate of $100 per player, and turn your form in via email at ryangirardot@sbabasketball.com, or you can do the transaction at the division home of your team.

Shamrock Basketball Association (SBA) 2010 Team and Player Agreement, Contract, and Waiver
This team, the ____________________________________________ in the _______________________ division of the Shamrock Basketball Association, playing at (facility name) ____________________________, hereby agree to all responsibilities, rules, and regulations set fourth in the SBA 2010 Captain’s Handbook, and agree to play the duration of the 2010 Shamrock Basketball Association season. All players on said team agree to void the Shamrock Basketball Association, said facility, officials, and league personnel of all responsibility for any injury, loss of property, or problems in any way. All signers of this document understand all payment received by the league and the facility are donations, which are in no way refundable. The current agreement and donation are valid for play throughout the 2010 SBA season. The SBA will provide jerseys, game officials, facilities, schedules, opponents, and final rulings in all league related matters. Any questions may be directed to ryangirardot@sbabasketball.com, or by contacting this facility. All schedules and league info are available at sbabasketball.com. The SBA, this facility, and all the teams across the SBA look forward to working with you this season.
